Before you begin, please have the following information handy.

 
 

  1. Emergency contact details

  2. Health Insurance, Doctor, and Dentist phone numbers

  3. A completed Student Waiver (required to register)

    • You will be required to upload a signed copy of the student waiver in order to register.

    • No student will be allowed on the mat without this.

    • If you don't own a scanner, you can use apps like Scanbot, Office Lens, or Scanner Mini on your smartphone.
